Falls remain one of the most challenging quality and safety concerns in long-term care. With increased regulatory scrutiny, facilities experiencing a high number of falls—or even a single resident with repeated falls—may be at risk for citation and potential designation as a Special Focus Facility.

Join us for this important webinar as we review current CMS guidance and discuss practical strategies to reduce fall risk, improve resident outcomes, and strengthen compliance efforts.

This program will focus on evidence-based fall prevention practices and interdisciplinary interventions designed to identify risks early, prevent avoidable falls, and promote resident safety.

1. Identify at least three key fall-risk factors and obtain relevant fall history information prior to or upon resident admission.
2. Develop a baseline care plan that incorporates individualized fall prevention interventions based on the resident’s specific risks and needs.
3. Implement resident-specific education and communication strategies that enable nursing staff and CNAs to consistently apply appropriate fall prevention interventions.
4. Integrate Physical Therapy and Occupational Therapy recommendations into interdisciplinary care plans to address identified fall risks.
5. Establish monitoring and supervision processes for off-shifts and other high-risk periods to improve resident safety and reduce fall occurrences.
6. Apply CMS guidance and survey expectations to strengthen fall prevention programs and reduce the risk of survey citations related to falls.
